 IV.     List the forms used in patient charts and describe their purposes.
 V.      Describe the organization of patient charts.
 VI.     Explain the importance of collecting health data on patients.
 VII.    Describe and demonstrate the procedure for obtaining a complete and accurate medical/dental health history on patients.
 VIII.   Describe and demonstrate the procedure for completing intra and extra oral examinations on patients.
 IX.     Explain the importance of obtaining vital signs on patients.
